Procedures for completing
an On-line Application
 

  1. You must answer all questions on the application completely and accurately.
       

  2. Information must be provided for both the mother and father of the children listed on the application.
     

  3. Bring the completed application along with a valid photo ID and Social Security card to the Admissions Department, 306 Reeves Court, Orlando, FL 32801, during normal business hours.  The Interviewer will review, validate and provide you with a copy for your records.
      

  4. If you move from the address on the application before you receive a letter from the Admissions & Occupancy Dept., it is your responsibility to come in to the Office or mail in your new address.  All changes to your application must be in writing.  No verbal changes will be accepted.
     

  5. When your name comes up for placement, an appointment letter will be mailed.  If the letter is returned because of an incorrect address your name will be removed from the waiting list.  Once your name has been removed from the waiting list you must re-apply when the waiting list re-opens.
     

IN ORDER FOR YOUR APPLICATION TO BE VALID, IT MUST BE STAMPED BY THE ADMISSIONS STAFF.
